How to Prepare for Your First Appointment
We’re looking forward to meeting you soon! In preparation for your first nutrition appointment, please be sure to complete both the Patient Information Form and the Practice Policies Form as thoroughly as possible sometime before the start of your initial appointment. These forms will be sent to you electronically via a link and access code through our HIPAA-compliant medical record system. Completing your new patient forms in advance of your appointment will allow us to prepare a bit before we meet so that we can best utilize our time together during your appointment.
New Patient forms
If you are unable to fill out the forms before your first appointment for any reason, they will be provided to you at the beginning of your first appointment.
Medical Records
It is very helpful if you are able to provide us with copies of your most recent blood work, physical exam, diagnostic test results, and/or any pertinent medical records prior to your first appointment as well. These documents can usually be easily obtained upon request from your primary care physician’s or specialist’s office.
Food Journal
If possible, please keep a food/beverage journal for approximately 3 days prior to our first visit, as this is our best assessment tool and the basis from which we're working. Please feel free to use a notebook, electronic device, or smart phone app to document your food/beverage intake - whichever is most convenient for you. We like the free smart phone application, MyFitnessPal, but any method of recording will work just fine. And if you are unable to keep a food journal prior to your first appointment, no problem. This is strongly recommended but not required.
OUT-OF-NETWORK OPTIONS for Nutrition Services
Because personalized care requires as much time and attention as is needed, Lauren Cornell Nutrition does not contract with any insurance providers at this time. We are a direct-pay practice with different fee options available. Additionally, your health plan may include out-of-network benefits that may make you eligible for reimbursement of some or all of your out-of-pocket costs. We suggest that you contact our insurance provider and inquire about reimbursement options for out-of-network medical nutrition therapy.
Upon request, we are happy to provide you with a Superbill which you may submit to your insurance provider for potential reimbursement of your costs.
Your Financial Responsibility
Payment is due upon arrival to your appointment. We accept credit card (Visa, MasterCard, American Express, Discover) and approved health savings account (HSA) and flexible spending account (FSA) cards.
